Magna Carta Health (MCH) is a preventive health services organization that uses clinical management and technology to prevent disease, disability and death. We specialize in providing disease management and prevention programs to individuals, employers, health maintenance organizations (HMOs), hospitals, and the government. Our goal is to improve health outcomes and quality of life from an individual perspective as well as across an entire population. We want to empower communities to be effectively involved in the provision of their own healthcare. Ours is a new approach to prevention and patient engagement that helps reduce costs while at the same time improve overall patient outcomes. We use mobile health technology with predictive modeling and analysis to bridge the healthcare gap.
